Payment Terms & Conditions
Effective Date: Sept. 1, 2021
Reveal Ultrasound Studio & Spa (“Reveal,” “we,” “us”) provides diagnostic ultrasound services that may be billed through insurance and managed by our billing office. By receiving services or making a payment toward a balance, you agree to the following Payment Terms and Conditions.
Billing Administration
Diagnostic ultrasound statements, patient balances, insurance processing, and EOB related questions are managed by Preferred Source Billing. Payments made to Reveal will be reported and applied to the patient account.
Payment Options
If you have received a statement, you may:
Pay online by entering the amount you would like to pay during checkout
Request a secure text payment link
Pay by phone when available
Mail a check (see below)
To help apply your payment correctly, we may require identifying information such as patient name and date of birth.
Payment Methods
Accepted payments may include credit or debit card, HSA or FSA, cash (in studio only), and check.
Checks by Mail
Checks must be made payable to Reveal Ultrasound Studio & Spa and mailed to:
1156 S. Bentley Blvd, Suite 1, Cedar City, UT 84720
Please do not mail cash.
Processing and Posting
Payments may take 1 to 2 business days to reflect depending on processing timelines.
Late Fees
Balances not paid by the due date shown on the statement may be subject to late fees. A late fee of 10% per month may be added to past due balances until paid in full, as permitted by applicable law. Late fees are assessed and reflected on your statement through our billing office.
Collections
If an account becomes seriously delinquent, it may be sent to collections. Accounts sent to collections may be subject to additional fees, including a collection fee up to 35% of the outstanding balance, as permitted by applicable law.

Separate Radiologist Bill
You may receive a separate bill from the radiologist’s office for the interpretation or reading of your exam.
Refunds
If you believe a payment was made in error, please contact us as soon as possible. Approved refunds are returned to the original payment method when possible and may take several business days to process.
Disputes
If you believe there is an error related to a payment made to Reveal, please contact us within 30 days of the payment date so we can review and assist.

We may update these terms from time to time. The most current version will be posted on this page.
